The 125-item Test Observation Form 2-18 (TOF 2-18) is a standardised form for rating observations of behaviour, affect, and test-taking style during testing sessions (individual ability and achievement tests) for children aged 2 to 18.
The TOF enhances the value of test scores by documenting test examiners’ observations of children’s behaviour during testing.
The TOF Profile is scored on five empirically based syndromes:
The TOF includes a DSM-oriented Attention Deficit/Hyperactivity Problems Scale and Inattention and Hyperactivity-Impulsivity subscales, comprising items identified by experts as being very consistent with DSM-IV diagnoses of ADHD.
The TOF may only be administered and scored by hand.
